INCIDENT WITNESS STATEMENT FORM
In the event of an incident involving a horse, rider, visitor, or property, this form is used to document observations from individuals who witnessed what occurred. Please write your account of the incident in as much detail as possible, focusing on what you saw and heard directly . Rider wellbeing is something I take seriously - and that includes having a clear plan in place when it comes to concussions. Falls don’t have to look dramatic to be serious, and even mild symptoms can affect a rider’s focus, coordination, and long-term health if they’re not properly addressed.
