INCIDENT WITNESS STATEMENT FORM

In the event of an incident involving a horse, rider, visitor, or property, this form is used to document observations from individuals who witnessed what occurred.

Please write your account of the incident in as much detail as possible, focusing on what you saw and heard directly. Stick to the facts - avoid personal opinions, speculation, or assumptions about causes unless clearly noted as such.
